Basic Advance Care Planning Course

Honoring Choices® Pacific Northwest, jointly sponsored by the WSMA Foundation and the Washington State Hospital Association, has partnered with Respecting Choices® to offer an online course to physicians and advanced practitioners in Washington state on basic advance care planning.

The Respecting Choices® Building Physician Skills in Basic Advance Care Planning course was developed with funding support from The Betty and Gordon Moore Foundation.

This 3-module curriculum will enhance physicians' and advance practitioners' ability to:

  • Effectively introduce and motivate patients to participate in basic advance care planning (ACP).
  • Guide and document basic ACP conversations with patients and families.
  • Successfully integrate and bill for ACP in everyday practice.

Includes downloadable tools and resources such as:

  • An ACP conversation guide to reinforce effective communication skills and ensure critical content is included in the conversation.
  • A 5-step design worksheet to identify workflow changes to integrate basic ACP into practice.
  • ACP billing resources that review appropriate use of ACP billing codes.
Who should attend: This three-module online curriculum is intended for physicians and advanced practitioners.

MODULE 1

Basic ACP: Introduce and Motivate

The purpose of this module is to identify the role of physicians and advanced practitioners in introducing patients to basic ACP and motivating them to participate.

At the completion of this online module, physicians and advanced practitioners will be able to:

  • Identify opportunities to introduce basic ACP in their practice for patients who have not started, or engaged in, the planning process.
  • Use communication skills and motivational strategies to increase patient participation in basic ACP.
  • Create a follow up plan for patients to have ongoing ACP conversations and complete their advance directive.

MODULE 2

Basic ACP: Guide and Document

The purpose of this module is to build physicians' and advanced practitioners' skills in guiding and documenting ACP conversations.

At the completion of this online module, physicians and advanced practitioners will be able to:

  • Guide a basic ACP conversation with patients and their health care agents.
  • Assist patients and their health care agents in discussing the three purposes of basic ACP.
  • Help patients create a written advance directive that reflects their goals, values, and preferences.

MODULE 3

Beyond the Conversation: Integrating ACP into Practice

The purpose of this module is to identify what additional information is needed for physicians and advanced practitioners to successfully integrate and bill for ACP in their practice.

At the completion of this online module, physicians and advanced practitioners will be able to:

  • Describe the steps to design a basic ACP program.
  • Apply strategies to develop a basic ACP program.
  • List the key components needed to bill for ACP conversations.

Continuing Medical Education (CME)

Accreditation Statement

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the Wisconsin Medical Society and Respecting Choices, A Division of C-TAC Innovations. The Wisconsin Medical Society is accredited by the ACCME to provide continuing medical education for physicians.

American Medical Association (AMA) Credit Designation Statement

The Wisconsin Medical Society designates this enduring material for a maximum of 2.5 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Maintenance of Certification (MOC) Part II

MOC Part II Credit Participants who wish to receive MOC Part II credit must complete all three (3) MD online modules to receive Part II credit. After actively engaging in the online modules, participants will be eligible to receive AMA PRA Category 1 Credit™. If you are seeking Maintenance of Certification (MOC) Part II credit from ABA, ABIM or ABS, please indicate your specialty board in the registration form.

NOTE: Not all specialty boards require the CME accreditor to submit participant completion data. The specialty boards listed below may not accurately reflect the MOC approvals for this activity. Please see the activity description below for MOC approval details.

  • American Board of Anesthesiology MOC Part II Credit

    This activity contributes to the patient safety CME requirement for Part II: Lifelong Learning and Self-Assessment of the American Board of Anesthesiology's (ABA) redesigned Maintenance of Certification in Anesthesiology Program® (MOCA®), known as MOCA 2.0®. Please consult the ABA website, www.theABA.org , for a list of all MOCA 2.0 requirements. Successful completion of this CME activity enables the participant to earn 1.5 MOC Part II Credit in the ABA MOCA 2.0 program. Maintenance of Certification in Anesthesiology Program®, MOCA® and MOCA 2.0® are registered certification marks of the American Board of Anesthesiology®.

  • American Board of Internal Medicine MOC Part II Credit

    Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1.5 MOC Part II Credit in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC Part II Credit equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

  • American Board of Surgery MOC Part II Credit

    Successful completion of this CME activity, which includes participation in the activity, with individual assessments of the participant and feedback to the participant, enables the participant to earn 2.5 MOC Part II Credits in the American Board of Surgery Maintenance of Certification (MOC) program. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABS MOC Part II Credits.
